UI/UX Designer Survey
INVESTIGATORS' STATEMENT
We are asking you to be in a research study. The purpose of this consent form is to give you the information you will need to help you decide whether or not to be in the study. Please read the form carefully. You may ask questions about the purpose of the research, what we would ask you to do, the possible risks and benefits, your rights as a volunteer, and anything else about the research or this form that is not clear. When all your questions have been answered, you can decide if you want to be in the study or not. This process is called ‘informed consent.’
PURPOSE OF THE STUDY
The purpose of this study is to learn about UI/UX designers and their knowledge of disability and accessibility.
PROCEDURES
To take part in this study, you must be 18 years of age or older, and be currently employed as a UI/UX designer, or similar. If you choose to participate in this study, we will ask you to complete a survey about your experiences as a designer, including your knowledge/experience with disability and accessibility. You will also have the chance to participate in a follow-up interview. It may take approximately 10 minutes to complete the survey.
RISKS, STRESS, OR DISCOMFORT
It is possible that you may experience emotional stress while answering questions about past experiences.
BENEFITS OF THE STUDY
You may not directly benefit from participating in this survey. However, your input may inform what designers know and what tools they use with respect to accessibility in their everyday work. Findings may inform future directions for research work. We plan to report our findings to the research community. Your participation is anonymous.

This study is not supported by external funding.
OTHER INFORMATION
Taking part in this study is voluntary and you may stop participating at any time. Your participation will be kept confidential. While we record contact information for the raffle and data integrity, your name will not be recorded with study data. Your name will not be used in any published or reported information. Linkage between your name, contact information, and the study will be destroyed at when the raffle has been completed, or after the follow-up interview, if you choose to participate in the interview. Data from this study may be used in future studies. On completion of the survey, you will be given an option to participate in a raffle as compensation. We will raffle $50 for every 100 responses received. If you wish to be included in the raffle, please enter your contact information. In addition, at the end of the study, we will also ask if you are willing to be contacted for a follow-up interview about your experiences with design and accessibility.
Government or university staff sometimes review studies such as this one to make sure they are being done safely and legally. If a review of this study takes place, your records may be examined. The reviewers will protect your privacy. Study records will not be used to put you at legal risk of harm.
