Supervisor verification for home delivery for fully remote faculty and staff

For faculty and staff who are fully remote, RIT Libraries IDS Department can mail items from interlibrary loan or from the RIT Libraries circulating collection to their homes.

To be eligible for this service, the staff members supervisor must fill out this form, attesting that the employee is fully remote and does not come to campus.

This form should be filled out by the employee's direct supervisor.

End date of when the employee will be fully remote (leave blank if the employee will always be fully remote) (mm/dd/yyyy)
